He’s starring in the upcoming film Mission: Impossible — Fallout , scheduled for release on July 27, and we know his impeccable acting chops and winning good looks will make the movie a great one. No matter how phenomenal he is as an actor, we can’t help but notice that Tom’s smile consists of a rather odd middle tooth .

While he has amazing pearly whites, Tom’s 100-watt grin has one tooth that’s just a bit out of place. You can see from the photo that his teeth aren’t exactly symmetrical with the center of his face. Many have noticed that the 55-year-old actor’s right front tooth is just a tad askew, and has been widely referred to as his “middle tooth” for quite a while now.

Long before Tom Cruise became  an ever-bankable action star , he was Steve Randle in  The Outsiders . And, amidst a cast full of fresh-faced brat pack hunks , it’s his unique set of chompers that set the young actor apart.

Cruise was injured in a hockey accident when he was young — hit by a flying puck — which left his front tooth chipped. And when filming The Outsiders , he decided to remove the cap that covered the crack. The result was purposefully unattractive, as Cruise meant to emulate his dirt-poor greaser character. And though The Outsiders was Cruise’s third movie, the 1983 film marked the first noticeable showcase for his unique mouth.

Corrective Procedures

As Tom Cruise’s career progressed, it’s clear that he received some orthodontic treatments. In addition to getting the original chip filled in, his teeth were whitened and straightened. Famously, in 2002, the then-40-year-old donned adult braces at the Minority Report premiere. Twenty years on, it may seem shocking; less noticeable options like Invisalign are now quite common. But for the time, Cruise’s bracket-less upper braces were actually a subtle choice.

Still, even after braces, Cruise’s signature smile was not quite perfect. His midline is off, creating the appearance of a so-called “middle tooth.” In the video above, a doctor from Premier Orthodontics breaks it down.

The average person has seven teeth on each side on each of their midline. But if you count Cruise’s teeth, it’s apparent that he’s missing the second pre-molar on his left side. (That would be the third tooth from the back.) Naturally, then, when Cruise lost his baby tooth the teeth around it would slide in toward that space… resulting in a midline that’s shifted roughly half-a-tooth toward the left.

And it’s not so uncommon — roughly 3% of the population has a missing tooth.

But since Tom Cruise is, well, Tom Cruise, the obvious next question is… Why doesn’t he get it fixed? The man has an estimated net worth of $600 million! But those options would require another, a longer period of more intense braces.

Open-coiled springs are one option. The small springs could be wedged into the braces, around the missing premolar, to slowly open up space for a dental implant to be added. In all, the process would take around two years. And two years as a metal mouth is a lot for a man who’s bringing in hundreds of millions at the box office — at age 59!

Regardless of the alignment of his teeth, or his missing premolar, one thing is for sure: Tom Cruise has the smile of a movie star.

While he knew he was handsome, Cruise was self-conscious about his height . "He said he would like some shoes that would lift him up a bit," she said.

Allan recounts being “speechless” because of the incident.

At that time, Cruise told Allan that he had lost his teeth after one of his children’s heels struck them while he was playing with them. While the actor had reportedly “pleaded” that the information not be made public, that, unfortunately, is not the case anymore.

It is believed that the Mission Impossible star had crowns on his teeth. However, it is possible for people to get both crowns and veneers at the same time.

What will suit them better will depend on the condition of their teeth, their location and the severity of their problem. But veneers can also effectively hide the teeth, although it only covers their front surface. 

Other than that, Tom Cruise may also have had a teeth whitening procedure.

Some people need these before getting dental crowns. Gingivectomy treatment involves the removal of gum tissue above the tooth so that more of its structure is exposed.

This provides a greater surface area to the crown for bonding and anchorage. It may be the reason why his gum line is not even anymore and why the size of his one tooth is now bigger than before.

It’s more likely that Tom Cruise has dental crowns instead of veneers. That’s because, unlike crowns, veneers only cover the front surface of the teeth.

And considering how damaged his tooth was, Tom Cruise’s dentist might have also recommended him a crown to protect the underlying tooth (provided it’s still there).

Tom Cruise remains elusive to this day. The actor hasn’t confirmed or denied anything about his teeth.

We only know that he had work done from his pictures and the fact that he very publicly got braces at the age of 40.

There are many rumours about the Minority Report star that he hasn’t ever said anything about. 

Despite being in the spotlight for more than 40 years, there isn’t much that people know about him. There was a time when people thought that he had a rhinoplasty or nose job.

However, in an interview with Playboy Magazine, he made it clear that it wasn’t the case and “never would” be. 

Did Tom Cruise Seek the Right Dental Procedures?

As mentioned before, Tom Cruise does have a million-dollar smile. His dentists would have tried their best to make what they could out of his situation.

It may not have been possible for him to undergo more invasive and extensive procedures because of the time constraints his career put on him. He has constantly been in the spotlight since the 80s. 

That said, the actor’s midline could have been better if he had had an implant after he had lost his tooth.

The implants would have acted as a substitute for the natural tooth and not allowed the surrounding teeth to shift. Plus, dental implants Turkey would cost him a little.

With a crown placed on top of the titanium screw, Tom Cruise’s smile teeth would have looked even more glamorous since his midline would have been at the centre of his face.

Of course, that would have been a problem once he started appearing in big movies, so he got his teeth fixed through cosmetic restorations like crowns or veneers or both.

According to Allen, Cruise invited him out one night and in the middle of their time together, two of the actor's veneers fell out of his mouth. In response, Cruise, who is well known for his affluent lifestyle , begged Allen not to reveal the story.

“They bought a house in the Village, and they wanted some privacy and they were worried about Page Six . So Tom wanted to have a drink with me, and you know, talk about his expectations."

“We went to a place — I think it was on 54th street, called Circo. We sat in a corner and he actually ordered a bottle of red wine, which I thought was surprising because he was a Scientologist and they generally don’t drink.”

“And we’re drinking and talking, and the next thing I know, both of his front teeth fell out onto the table cloth right in front of me.”

“I mean, this is a face that’s on posters all over the world, good looking guy and there are his front teeth."

“I made the mistake of telling Cindy, who you know [said] ‘Oh my god that’s great, I’m gonna write that,’" he recalled, "and I said, ‘No you can’t do that. I gave Tom my word I wouldn’t do it.’"

The Top Gun 2 actor, who turns 60 in July, was interviewed by Phillip Schofield  and Julie Etchingham during the Queen's Platinum Jubilee Celebration.

Another wrote: "Crikey, Tom Cruise looks like Tom Cruise again" - after photos of the star at a baseball game in San Francisco last year showed him sporting a much fuller-face.

At the time fans questioned whether he'd had botched fillers, with one tweeting: "What has Tom Cruise done to his face?"

With his boyish good looks and absence of frown lines, plastic surgery rumours have dogged the actor for years.

Although he has consistently denied going under the knife, fans have speculated the ever-youthful star has had a nose job, hair transplant and Botox, as well as veneers on his teeth.

But when leading man status beckoned, with roles like Maverick in Top Gun, he had his teeth whitened to improve the look.

The superfit star pushed himself to the limit whenever he was filming a stunt-filled action role but would also put on a few pounds between movies.

Speculation about Tom having surgery began in 2012

Rumours the star had undergone a nose job began back in 2012 but, asked by Playboy magazine if he had ever had surgery, he replied: “I haven’t and never would.”

But pal Cuba Gooding Jr fanned the flames by insisting Tom had “absolutely” had work done.

Cuba told a US TV show: "I don’t know what he’s had done, but I remember I surprised him at his house one day and he had all of these dots all over his face.

"I was like, 'You all right?' and he goes, 'I didn’t know you were coming' and I was like, 'I can see why.'"

Alice Henshaw explained the dots could indicate many types of surgical or non surgical procedures to make the skin tighter and younger-looking.

She said: "Often dots or markings are drawn on whilst we mark out injection points on the face, but we do this for filler injectables, Profhilo (a collagen stimulator) or even a skin tightening procedure using radio frequency such as Thermage, or a medical grade ultrasound skin tightening like ultraformer."

Tom appeared 'puffy' at the 2016 BAFTA awards

Tom's appearance at the 2016 BAFTAs had fans commenting on how he looked "inflated" and "puffy" - a common sign of both Botox and fillers.

"Tom looked as if he had undereye filler and filler around his mouth and nasal labial fold in 2016," said Alice.

"But the recent pictures at the baseball match show swelling that doesn't come with fillers."

Dental experts also suggested he had veneers added to make his dazzling smile even whiter.

At the Mission Impossible: Fallout premiere in 2018, Tom looked leaner, with more lines showing on his forehead.

While this could be down to a punishing exercise regime and work schedule for the film, which he also produced, it could also mean he ditched fillers and Botox after the 2016 pictures attracted unwanted attention.

Snaps of the star at a baseball game last year raised eyebrows once again. They came hot on the heels of snaps of Tom at a Birmingham curry house where his face also looked "puffy".

Alice said it could be Tom - like many of us - just put on a few pounds during lockdown.

"It would be hard to say whether it was weight gain without seeing full photo of his body," she added.

However he didn't appear to be carrying excess weight during summer appearances at Wimbledon and Wembley.

Alice said the Mission Impossible star looks "swollen" - potentially after going under the knife.

"Tom definitely looks swollen and puffy, which could be due to filler, post procedure swelling or weight gain," she explained.

"I believe he's had fillers under his eyes and around his mouth and nose in the past, but this time is different.

Why Tom Cruise Fans Think The Star Had Plastic Surgery

Tom Cruise has long been deemed one of Hollywood's most desirable men, but even that has not exempted him from plastic surgery rumors. This has been primarily due to his changing looks over the years, such as a possible decrease in wrinkles, which some have attributed to Botox. Fans have debated his procedures online, with one X user speculating, "Tom Cruise has either gotten botox done or plastic surgery...not hating, just being honest." Another even pleaded with him to ease up on changes to his face, writing "This is painful but, I'm just going to say it....#TomCruise needs to stop with fillers, botox and lifts."

Theories such as these were only fueled when Cruise showed up at a San Francisco Giants and Los Angeles Dodgers game in October 2021 with a visibly swollen face. His shocking appearance led many to believe he was either recovering from a cosmetic procedure or had gained quite a bit of weight. One month later, he was spotted with his face seemingly back to normal, though that didn't seem to ease the whispers about a possible nip tuck.

Several Surgeons Have Suggested That Cruise Had Multiple Procedures

After Tom Cruise was spotted at the baseball game, Dr. Frank Agullo expressed his thoughts about the actor's face to Radar Online . "Tom Cruise has always seemed to defy aging, but in this photo, it seems like he has gained facial weight or had too much fillers to the cheeks," he explained. Another surgeon weighed in after Tom Cruise stepped out in Miami for the May 7, 2023, Miami Formula One Grand Prix, with what appeared to be a more chiseled jawline.

Plastic surgeon Dr. Richard Westreich of New Face New York, broke down why this may have been, telling The U.S. Sun that he believed Cruise could have spent upward of $50,000 to improve his face. "There's the possibility of functional rhinoplasty, of laser skin, resurfacing, and perhaps even a little lipo under his chin," he explained. Neither professional has worked on Cruise, so their observations are only their expert opinions. While there could be some validity, the Oscar nominee previously shared that he would never undergo such enhancements.

Cruise Once Stated He Wouldn't Get Any Cosmetic Work Done

The former husband of Katie Holmes  sat down with Playboy Magazine in May 2012, where he answered a series of questions. Cosmetic surgery came up in the chat. When asked if Tom Cruise ever underwent any beauty alterations, he replied, "I haven't, and I never would." It's possible he could have had a change of heart in the years that followed, though the relatively private action movie star may never confirm nor deny the allegations.

While such theories have plagued the "Mission Impossible" star, they are certainly not the most scandalous controversies of Cruise's career . There has been no concrete evidence to suggest Cruise has gone under the knife. Still, the rumors likely won't stop as he continues to age.
