Had my itinerary printed before I got to my Florida destination, got there and the hotel reservation was non existence company travel, meaning I have to go through National Travel, well it was dark they had no rooms for that price called NT, the booked me another hotel a mile and 1/2 away NT gave me the reservation number mind you it night and roads are wet. I finally found the hotel go in give the reservation number front desk says ma'am we don't have a room for you, I'm like really, I literally had to reach out to a co-worker to help me find and secure a room. Called NT, and they said let me find you something in the area, I said NO, I'm in the lobby at a hotel who have room you need to booked me a room and give me the confirmation number, I will need you to stay on this line until the reservation is complete! I don't recommend paying them for this type of service.

Friendly, but not very helpful. Tried getting some quotes on vacations and the agent kept sending me only half the information, no flights included in anything. Very frustrating. I ended up calling vacationexpress.com directly and got everything I needed in just a few minutes and in some cases at half the rate these guys were quoting. I think this is more for the business or corporate group traveler. The difference between someone who took the time to understand my group's travel needs and someone who didn't was eye opening.

National Travel will give you the cost that you should fill in on the CJA Travel Request & Authorization (.pdf) under Airline Fare. If National Travel is not used to obtain your airline ticket, reimbursement is limited to the authorized government rate .
  • You can also obtain car rental reservations through National Travel. They will give you the cost to fill in on the CJA Travel Request & Authorization (.pdf) under Rental Car/Daily Rate.
  • To obtain the per diem rate for a certain destination for daily lodging and meals, please visit the U.S. General Services Administration website (www.gsa.gov …) . Click on the map for your travel destination. Fraudulent Email Alert

We are aware of an individual fraudulently claiming to represent ITA in order to solicit personal information. Individuals have reported receiving emails and phone calls “regarding a possible recent scam involving your vacation property.” The recipient is then instructed to complete a complaint form with personal information to receive support from ITA.

This is a scam and was not sent by ITA or any of its employees. If you receive such a communication, please do not respond or provide any personal information. 

If you believe that you have been a target to such a scam, please notify the Department of Commerce, Office of Inspector General, immediately and file a report.

How to Apply at a Passport Agency

1. find out if you're eligible.

To apply at this passport agency, you must:

  • Have international travel within 14 calendar days.
  • Show printed proof of international travel. See examples in the 'Collect all your application materials' section below on this page.
  • Pay the $60 expedite fee with your  application fees .
  • Note : If you need a foreign visa, your appointment must be within 28 calendar days of your travel.

If you have a life-or-death emergency, please see  Life-or-Death Emergencies . If you don't meet the requirements for an appointment, request expedited service .

We do not charge a fee to make appointments . If you asked to pay for an appointment, you should consider the request to be fraudulent. We are not affiliated with any third-party appointment booking services. We may not be able to honor appointments booked via third parties. Do not attempt to book duplicate appointments.

2. Collect all your application materials

  • Printed confirmation of your appointment
  • Flying : show a flight receipt or an itinerary.
  • Driving or taking a boat to Canada, Mexico, Bermuda, or the Caribbean : show a hotel reservation, cruise or bus tickets, or international car insurance.
  • Fill out your  form(s) online and print it  
  • All required supporting documents
  • One passport  photo
  • Major credit cards
  • Debit/check card (not ATM cards)
  • Digital wallet payments such as Apple, Google, or Samsung Pay
  • Checks or money orders. Make payable to the U.S. Department of State.
  • Cash (Exact amount only - no change provided)
  • If you are applying for a child under 16 , both parents or guardians and your child must come to the appointment.

3. Make an appointment

Follow these steps:

  • Go to our Online Passport Appointment System .
  • Enter information about your travel plans to see if you qualify for an appointment. You may make an appointment for up to 7 members of your household.
  • Enter your email address. We will email you a code which you must enter to continue making an appointment. Check your junk or spam folder if you do not see the code in your inbox.
  • Enter your mobile phone number. We will send you an SMS text message with another code you must enter to continue making an appointment.
  • Finish scheduling your appointment. Check your email for a message confirming the details of your appointment. The system will hold your appointment for 15 minutes. If you do not confirm your appointment within 15 minutes, start over.
  • If you need to cancel or change the appointment, click the link in the confirmation email to get information about your appointment. If you made an appointment over the phone, forward the email you received confirming your appointment to [email protected] with “Cancel my Appointment” in the subject line.

4. Arrive early to your appointment

  • Arrive 15 minutes early to go through security screening. 
  • You cannot bring weapons, food, and drinks (except water in a sealable bottle).
  • Check in at the information window. Give them your appointment confirmation number.
  • Be seated in the lobby and wait until we call your number. We may not see you exactly at your appointment time.
  • When we call your number, go to your assigned window.
  • At the window, we will review your application and supporting materials. We will discuss options for picking up or delivering your passport.

National Travel Assistance

What is national travel assistance.

The National Travel Assistance Scheme helps people who need to travel long distances or travel frequently. You need to be referred by your specialist to see another specialist. The specialists must both be part of a government-funded health and disability service.

The Scheme is administered by Health New Zealand | Te Whatu Ora who uses the NTA policy to guide their decision-making according to the eligibility criteria.

The policy on which the scheme is based is the National Travel Assistance (NTA) Policy 2005.

Who can get travel assistance

You may be eligible for the National Travel Assistance Scheme if all of these apply.

  • You have been referred to the specialist you are going to see by another specialist (not a GP).
  • Both specialists are publicly funded. This means they are part of a government-funded health and disability service — for example, a renal dialysis centre or a specialist disability service.
  • You can answer ‘yes’ to one or more of the questions in the checklist below.

National Travel Assistance questionnaire

  • 80km one way per visit (for a child)?
  • 350km one way per visit (for an adult)?
  • Do you visit a specialist at least 22 times in 2 months?
  • 25km one way per visit (for a child)?
  • 50km one way per visit (for an adult)?
  • 80km one way per visit (for an adult)?

Who can not get travel assistance

You may not be eligible for assistance under the National Travel Assistance Scheme if you receive or are eligible for travel assistance from another provider, such as:

  • Work and income
  • Ministry of Social Development
  • Ministry of Transport
  • Ministry of Education.

What travel assistance covers

Travel costs.

Assistance with travel costs may cover:

  • private vehicles
  • public transport
  • specialised transport needs — for example, taxi, mobility taxi or air travel.

The mileage is calculated by the shortest practical route from your residential address to the facility or hospital treating department.

Accommodation costs

Accommodation costs may include payments towards:

  • the cost of a motel room
  • allowance when staying with family and friends.

Accommodation is not usually approved for travel less than 100km one way.

Support person’s costs

You may be able to claim assistance towards a support person's costs. This is when the specialist recommends you have a support person to assist you with clinical decision-making or provide physical support.

When a child is eligible for travel and accommodation assistance, they are automatically eligible for assistance towards a support person's costs.

Specialist approval

Your specialist may need to approve claims for some of these costs (for example, accommodation, support person and specialist transport).

How to claim travel assistance

Get registered.

  • Your hospital travel coordinator, health or disability specialist, or nominated social worker will need to help you register for travel assistance. They will send the completed registration form to Health New Zealand | Te Whatu Ora.
  • Health New Zealand will assess your application.
  • If you are eligible, a confirmation letter and blank claim forms will be sent to your mailing address.

File a claim

  • Complete each claim form.
  • Get it signed and stamped by the facility or hospital. You can also attach a signed and stamped proof of attendance. This can be a hospital appointment discharge card, letter or note, on hospital letterhead paper.
  • Attach your receipts. They must be original and itemised.
  • When you make your first claim, attach a deposit slip, the top of your bank statement, or account verification from your bank. If your bank account details change at any time, repeat this with your new bank account details.
  • For some claims, we ask you to wait until the minimum number of visits has been attended before submitting your first claim. 
  • You must put in a claim for travel assistance within 12 months of the date of the appointment.

Any personal information you provide when registering or making a claim for travel assistance will be held securely by Health New Zealand. This will be kept confidential, except for any disclosure that may be required by law.
