Home Visit Program

It is not easy for some patients to see their family doctor. They might have trouble walking. They might not be able to take the bus or drive. They might be too sick to leave the house.

If you are a South East Toronto Family Health Team (SETFHT) patient and you cannot leave your house, we will come to you!

You are part of the team and we will work together to plan your care and answer your questions about your health. We can also help you with other issues that might affect your health and well-being, such as meals and housing.

We want to work with you to keep you as healthy as possible.

If you are not already a SETFHT patient and want to join our Home Visit Program, ask your doctor or Home Care Coordinator to complete the Home Visit Referral Form and fax the completed form to the attention of  Vanessa De la Vega , Programs & Services Assistant, at  416-423-8803 . Getting to the doctor’s office can be a problem for many seniors so a number of services are popping up across the country to provide house calls either in person or through video-conferencing for non-emergency medical needs. Most medical services are covered by provincial health plans.

Here is a sampling of home visit providers in major centres across Canada that you can consider to determine if they may be right for you or a loved one.* Most are not intended to take the place of a family doctor but provide after-hours care when your doctor is unavailable.

Toronto and Greater Toronto Area

National Home Doctor Service

  • More than 100 doctors offer after-hours house calls when your family doctor is unavailable, evenings, weekends and public holidays
  • For patients with acute, episodic conditions (e.g. vomiting, diarrhea, UTI, respiratory infections, Influenza)
  • Services are billed to OHIP with a valid health card
  • Bookings accepted 8am-midnight, 7 days a week

Details of Services

Our walk in clinic fills the gap in between the emergency room and doctor’s appointments. Visit the walk in clinic if you have a non-emergency issue needing timely care, and you are unable to meet with a doctor.

Urgent Visits for Illness/Injury

We provide care for your common ailments including coughs and colds, respiratory tract infections, allergies, joint sprains, muscle aches, suturing of simple cuts, urinary tract infections and more.

We provide general medical services as expected of a family physician including assessment of presenting symptoms, investigations, treatment and referrals when necessary.

Periodic Health Examinations (Previously called Annual Physicals)

This is a detailed assessment of your general health, including preventative and screening health measures like the stool test (for colon cancer), colonoscopy (for colon cancer), PAP test (for cervical cancer), mammogram (for breast cancer), BMD scan (for osteoporosis).

We provide the whole range of vaccinations for both children and adults. Also ensure you ask for your yearly flu shot during the winter months.

We offer home visits for selected patients and at the doctor’s discretion. Kindly make inquiries by phoning the reception if you require one.

We know that elderly patients require their own unique care as well as sufficient time to address their concerns. Kindly request a double appointment if you think you will need more time.

Women’s Health

We will advise on and prescribe a wide range of birth control methods, ensuring that it’s the right one for you. Where appropriate, we may refer you to a specialist.

Once you find out you are pregnant, book an appointment with us. We will go over the blood tests and scans required and arrange regular appointments throughout the initial stages of your pregnancy until your care is handed over to the specialist or midwife of your choice.

We carry out well-baby checks where we ensure babies are meeting their developmental milestones as well as addressing any health concerns at the time. During these visits, we also carry out detailed examinations and ensure their childhood vaccinations are up to date.

We will also carry out post-natal care in the few month after your delivery to address any issues that may have arisen during your delivery as well as guide you through the commonly encountered problems after delivery.

The TB skin test is used to detect infection with TB bacteria. The TB test consists of two appointments.

Allergy Shots

Help your body get used to allergens, the things that trigger an allergic reaction.

Freezing and other remedies to treat your warts.

Our doctors are registered with WSIB and will carry out the relevant examinations and treatment after your workplace injury. We will also complete the relevant paperwork.

A physical exam is a routine test done by the doctor to check your overall health, to ensures wellness and good health to provide to an employer.

Driver’s Medical Examinations

We carry out drivers’ medical examinations and complete the relevant paperwork required by the ministry. This service is not covered by OHIP and so there is a minimal charges.

 Travel Clinic

We will advise you on the vaccines and protection required for your travel depending on your destination country. This service is not covered by OHIP. As such, there is a small fee charged

Find a family doctor or nurse practitioner.

A family doctor or nurse practitioner is your primary health care provider – meaning they are the person you make an appointment with when you have a new, non-emergency health concern. You can learn more about their services below .

Visits to a doctor are publicly funded so you don’t have to pay.

Some nurse practitioners work in clinics that are publicly funded so you may not have to pay. To find out more, ask the clinic about payment before you book an appointment.

Here are two options to find a family doctor or nurse practitioner who is accepting new patients:

  • Register with our Health Care Connect service and have a nurse find a doctor or nurse practitioner for you.
  • Use The College of Physicians and Surgeons of Ontario’s Find a Doctor search .
  • Choose “ Advanced Search ” to find a doctor near you (by city/town or postal code).
  • Click on “ Additional Search Options ” to narrow your search (to family doctors and/or language spoken).
  • Contact the doctor to check if they are accepting new patients.

What family doctors and nurse practitioners can do

A family doctor and a nurse practitioner can provide:

  • diagnosis and treatment for common illnesses and injuries
  • referrals to health care specialists who can help with a specific condition
  • support in managing a chronic condition (such as diabetes or high blood pressure)
  • prescriptions for medication
  • regular check-ups including physicals and routine screening tests ( e.g. , for cancer)

Have a nurse find you a doctor or nurse practitioner through Health Care Connect

Register for Health Care Connect and a nurse will search for a doctor or nurse practitioner who is accepting new patients in your community.

Or register by phone at  811 , 24 hours a day, 7 days a week.

About 80% of people who join the program find a family health care provider this way, however, joining Health Care Connect does not guarantee that:

  • a family health care provider will be found for you – those who need urgent care will be placed with a family health care provider first
  • one family health care provider can take your whole family

Who can use the program

To use Health Care Connect, you must:

  • not currently have a family health care provider (learn how to remove yourself from your current provider’s patient list )
  • have a valid Ontario health card
  • have an up-to-date mailing address associated with your health card ( here’s how to update it if you need to)

How to join the program

You can sign up for yourself and your whole family or for someone else (with written permission).

To sign up for the program, you’ll be asked for your name, health card number and some basic health information that will help determine your health needs.

There are two ways you can give this information and sign up:

  • Visit Health Care Connect online at any time.
  • Call  811 24 hours a day, 7 days a week. Select your language preference and press 2 to find a doctor or nurse practitioner accepting new patients in your community.
  • Review your request.
  • Try to find you the right family health care provider in your community who is accepting new patients.
  • Contact you if a provider is found for you, and give you their contact information.
  • Contact the provider that’s been found for you as soon as possible – if you choose not to make them your provider and want Health Care Connect to keep looking, you will have to register again.

If you currently have a family health care provider

You can only join Health Care Connect if you don’t have a family health care provider. If you would like to use Health Care Connect to switch to a new family health care provider, you must first take yourself off the patient list of your current one. There are two ways to do this:

  • contact your family health care provider directly
  • call ServiceOntario –  1-888-218-9929 ( TTY : 1-800-387-5559 )

The types of injuries and illnesses we treat at the Urgent Care Centre include infections, earaches, eye injuries, sprains, broken bones, cuts, fevers, minor burns, phlebotomy (vein puncture), stitches (sutures and staples) and blood lab services.

Your conditions will be managed professionally, safely, and with the utmost care. The HealthOne Urgent Care Centre is available if you need treatment for illness or injury that is not life-threatening. Our team also offers follow-up appointment scheduling to monitor your progress and reassess treatment options if necessary.

You will be required to show your Health Card at every visit. If you do not have a Health Card, our team will guide you through the registration process and be happy to assist you.

We also accept UHIP, MORE Care and CIGNA TT Care Cards and direct bill to your provider.

Symptoms: Loss of consciousness, pain remaining after taking pain medication, confusion, persistent and severe chest pain, difficulty breathing.

  • Receive care for acute conditions such as illness or injury
  • Shorter wait times than the Emergency Room for non-life-threatening cases
  • Follow-up appointments available after your visit

  • Sexual Health & Pregnancy (i.e. pap smears, Urinary Tract Infections (UTIs), birth control, emergency contraception (morning-after contraception), pregnancy tests, sexually transmitted disease (STD) testing and treatment
  • Minor procedures that can be performed under local anesthetic

COVID-19 Rapid Antigen Screening Test (TRAg) and Other Services

Rapid antigen screening is a screening tool that can identify antigens in your body. Antigens are proteins that can be found on a virus, which may suggest that you have an active COVID-19 infection. In order to confirm your COVID-19 diagnosis, you must complete a COVID-19 polymerase chain reaction (PCR) test. Rapid antigen screening is not a replacement for public health measures including symptom screening, maintaining proper physical distance, wearing a mask, and practicing proper hand hygiene. Please speak to a doctor for more information.

Your walk-in clinic visit will be covered by the Ontario Health Insurance Plan (OHIP) if you have a valid OHIP or a health card from all other provinces except for Quebec. We also offer direct billing for UHIP, Morcare, and Guard.Me. Certain services including medical notes, travel consults, and vaccinations are not covered by OHIP. Yes, we offer a variety of different vaccinations at our clinic including bacteria and virus vaccinations, allergy shots, birth control shots, flu shots, skin tests and urine tests. It’s recommended that you book an appointment with your family doctor first. This helps make sure you receive continuous care from your healthcare provider. In the case where your family doctor is not available, you can visit our Medical Walk-In clinic for an appointment.

Unfortunately, our family doctors are currently not accepting new patients nor does our clinic have a waitlist for new patients. Medvisit provides after hours medical care to patients around Canada. In the evenings or on the weekends, our Doctors are on the road, bringing medical care to patients suffering illness and injury that are unable to leave their home to access care.

More than half the calls we receive are made not by the patients themselves, but by carers. They may be the worried parents of sick children, the husband or wife of a sick partner, the carer of an ageing relative, or the nurses or clinical managers in an senior care facility.

It is always very worrying when a loved one falls ill in the after hours, and our Doctors are very aware of this. Along with their specialized clinical skills, our Doctors are well trained at the time-honored skill of the “bedside manner” - lending a sympathetic ear, and calming anxieties the patient or caregiver may have.

Our Doctors are also very aware of the special needs of patients, from treating infants and small children, whose symptoms often get worse at night, to treating patients with a disability, or helping families with ageing relatives. 

Time after time our patients write to us and tell us how wonderful our Doctors are. We know! Because our Doctors are able to give the expert care, kindness and reassurance that you would expect from your family doctor, but in your home environment, in the evening, late at night, or on the weekend, when Family Practices are closed.  

Medical care

Our Doctors treat common, acute conditions such as respiratory infections, migraines, gastro, ear infections, fevers, urinary tract infections, and injuries from falls: conditions that, while not life-threatening, require urgent medical attention. All visits are covered by your Provincial Health Insurance (e.g. OHIP)

All Doctor home visits are covered by Provincial Health Insurance programs, for patients with a valid Health card that meet the criteria for frail elderly or housebound. 

Not for emergencies

Medvisit is a service providing medical care to patients in the after hours. It is not a service for medical emergencies. If symptoms are critical or life-threatening, you should call triple 911 or go straight to hospital. Nor does Medvisit treat requests for routine medical care. All routine medical treatment, such as prescription repeats, immunizations, referrals, check ups or chronic disease management, should be handled by your family doctor.   

Continuity of care with your family doctor

After the home visit, Medvisit advises that you take our Doctor’s medical report to your family doctor, so they are kept in the loop. You can also request we send it to your doctor for you. This is important to maintain continuity of care, because your family doctor knows your medical history, and is the best person to manage the ongoing health of you and your family.
